The majority of our packages ship USPS Priority Mail, as we have found their flat rate to be least expensive in most cases. However, on the west coast, UPS Ground may be less expensive. To determine which method is best for you, it would be better to separate the package price of the CMS 1500 forms from the shipping. To do this, go to this page: pd_cms_1500_form_laser_packages.cfm. Add the quantity of packages you are interested in to your shopping cart. On the shopping cart page, there is a shipping calculator. This calculator displays USPS and UPS rates*. Packages of HCFA/CMS 1500 forms ship out of our Washington (state) location. USPS Priority Mail service is a 2-3 day delivery time. UPS takes 1-5 work days, depending on what zone you're in from WA. *UPS rates displayed in the shipping calculator are based on Commercial shipping locations. To get the residential UPS rate, you will need to proceed to the next step in the checkout process and enter your address information as if you were checking out. You will not be asked for your credit card information until after you have seen all possible charges for your order.
Carton orders
Our cartons of HCFA/CMS 1500 forms ship out with a guaranteed UPS rate of $8.00 per carton. These cartons are 30 pounds, and $8.00 is an excellent rate with UPS. Normal UPS shipping rate on this weight ranges from $12.50 - $15.00 depending on where you're shipping from/to. These CMS 1500 forms ship out of the nearest plant location: Ohio, New Jersey, Georgia and California.
