form dated (02/12) has expired. This is a standard practice within the Government Agency, Office of Management and Budget
(OMB). The agency is currently in the process
of extending the OMB number 0938-1197 with a new date of (09/19) within
internal departments. No changes to the forms fields have been made,
however the date change at the top and bottom of the from has been
updated from (02/12) to (09/19).

CMS 1500 Forms For Laser Printers
Laser forms are printed on very special paper. Please, do not print your CMS 1500 claim forms on blank white paper. Typical blank paper will not scan correctly. Many major carriers scan the forms you send to them. Forms that cannot be scanned are slowed down in processing or rejected altogether. Our CMS 1500 laser printer forms are printed on special, hi-scanabilty paper. We guarantee our laser forms will scan.
CMS 1500 Forms For Pinfeed Printers
Pinfeed forms are available in 1,2,3 or 4 parts. Multiple part forms are pinted on carbonless, black printing paper. Like our laser forms, we guarantee our pinfeed forms for scanability.
CMS 1500 Hand Written Forms
Hand written forms are designed to be "snapped" apart after use. These forms are not compatibel with any standard printers. Sometimes called snap-a-part claim forms. Scanability is also gauranteed.
