Just CMS 1500 Forms.com is a division of genusSystems.

We have been servicing health care professionals since 1979.  Our management team includes professionals with several decades of experience with heal claim forms, envelopes, statements, super bills, filing systems and other healing arts office supplies. We launched JustCMS1500Forms.com in November of 2006 to provide health care professionals with current, reliable and affordable online site to order cms 1500 forms, cms 1500 envelopes and related suppliers.

Mark Thompson  CEO

Mark Thompson is the founder and senior partner of our parent company, genusSystems. He began his career in 1979 working as a health care sales representative for POS, Inc (Professional Office Services) in Waterloo, Iowa.  Mark stayed with POS until 1989, earning the respect of his customers and colleagues for innovating new services and products to help healing arts offices across the nation run more efficiently and more affordably. Mark held senior level positions in the office printing and products industry throughout  the 1980’s and 90’s. He started genusSystems in 1980 as a “platform” company to develop and implement internet based solutions for health care office and vendors. 

Mark’s career accomplishments include, Founder of genusSystems, Founder of Brochure in a Box™, Co-Founder of genusWorx, VP of Business Development for Bear Rock Technologies, seven time Master’s Circle Award winner and Chairman of President’s Council for POS, Inc., Board Member Mark-It Smart, and print-enabling technologies guest speaker for the PSDA, DMIA and the IBFI. He holds a dual BS degree in Education and English from Iowa State University.

Lisa Thompson  COO

Lisa Thompson heads up the Just CMS 1500 Forms business unit of genusSystems. Lisa began her career in 1985 working as a receptionist for Great Lakes Business Forms in Muskegon, Michigan.  She accepted a position with Thomas Business Forms in Camarillo, California in 1986 (now Cenveo, Inc.). Demonstrating a keen commitment to “customer first” service, Lisa was rapidly promoted from receptionist, to billing clerk, to estimator, to customer service representative and ultimately to customer service manager.

In 2006, after getting her youngest child into school, Lisa returned to the printing industry, joining JustCms1500Forms as customer service representative. She has been promoted since to customer service manager and to COO. Lisa work with customers, still, everyday making sure that her commitment to “customer first” service is delivered with every customer interaction.

Lisa’s career accomplishments include Customer Service Manager of the Year - Poser Business Forms, Camarillo, CA (now Cenveo), AMA certificate for Customer Service.
